There are so many medical shows out there. More than enough, apparently, to keep Americans fully booked on emergency rooms, impossible diagnoses, emotional hallway speeches, and doctors who somehow have time to save lives and unravel their personal lives in the same episode.

Still, there’s a reason viewers keep coming back. Medical dramas know how to raise the stakes, and the fictional doctors at the center of them are often what make those stories stick. Some are brilliant, some are chaotic, and some are probably best remembered for making things worse, but all of them have left a mark on pop culture.

That got us wondering which of these fictional doctors sticks out the most in our collective consciousness? To find out, we analyzed search interest over the past 12 months, comparing 50 recognizable fictional characters with “doctor” in their title to determine the most popular one in each state. To be honest, we played pretty fast-and-loose with what a fictional doctor is, but that seemed more fun.

The results revealed a fun mix of classic characters, modern TV favorites, and a few unexpected picks. Below, we’ll break down which fictional doctors came out on top around the country and what their popularity might say about the stories Americans still can’t get enough of.

Who Is Your State’s Most Popular Fictional Doctor?

From primetime prodigies to total cartoon disasters, doctors in fiction come in all shapes and sizes. To find out which ones Americans are most interested in, we analyzed regional search interest to identify the most popular fictional doctor in every state.

A few characters managed to stand out more than the rest, claiming multiple states across the country. Some make perfect sense based on just how iconic they are, while others seem to reflect the lasting pull of specific shows, stories, and personalities.

These are the fictional doctors that came out on top of our list:

1. Dr. Nick  (The Simpsons

  • Most popular in seven states: California, Colorado, Connecticut, Massachusetts, New York, Ohio, and Washington.

Dr. Nick may not be the doctor anyone would actually want treating them, but he’s easily one of the most memorable. With his instantly recognizable one-liners and long-running role in one of America’s most famous animated series, it’s not too surprising that he led the pack. 

2. Dr. Watson (Sherlock Holmes)

  • Most popular in five states: Idaho, Maryland, North Carolina, Tennessee, and Wisconsin.

Watson brings a very different kind of appeal. He’s steady, intelligent, and tied to one of the most enduring fictional worlds ever created. While Sherlock Holmes usually gets the spotlight, Watson has remained a beloved character in his own right for generations. 

3. Victor Frankenstein (Frankenstein)

  • Most popular in four states: Delaware, New Mexico, Texas, and Virginia.

Frankenstein is probably the darkest entry among the top names, but he also comes from one of the most influential works of horror fiction ever written. He’s not the warm, reassuring kind of doctor viewers root for in a hospital drama, but that’s part of what makes him so interesting. And please don’t make the common mistake of thinking the monster’s name is just “Frankenstein.”

4. Dr. Will Halstead (Chicago Med)

  • Most popular in three states: Illinois, Missouri, and South Carolina.

As one of the central faces of a major network medical drama, Halstead has the advantage of being part of a franchise with a loyal audience. His win in Illinois feels especially fitting given the show’s Chicago setting. That home-region connection likely gave him a boost, while the broader success of Chicago Med helped carry him beyond the Midwest.

5. Dr. Neil Melendez (The Good Doctor)

  • Most popular in three states: Arizona, Arkansas, and Florida.

Melendez made a strong showing for a character from one of the biggest medical dramas of the past several years. As a skilled surgeon with a complicated personal side, he fits the mold of the kind of character viewers tend to connect with in ensemble hospital shows. His popularity shows that newer TV doctors still have plenty of power to compete with longer-established names.

Overall, the leaders were a mix of comedy, classic literature, and modern medical TV. This study is a reminder that when it comes to fictional doctors, Americans have more than enough to choose from. Closing Thoughts

Whether they’re brilliant, chaotic, compassionate, or completely questionable, these “doctors” sit at the center of the high-stakes stories that make medical shows so hard to turn off. Part of the appeal is the drama, of course, but it’s also the people in the scrubs who keep viewers invested week after week.

The doctors may be fictional, but the themes behind their stories are anything but. Medical dramas are built on life-changing decisions, pressure-packed moments, and questions of trust, which are the same kinds of issues that shape real conversations around patient safety, accountability, and medical malpractice.

For Weiss & Paarz, that connection matters. While this study takes a light-hearted look at the fictional doctors Americans love most, it also reflects the real-world importance of medical care being handled with skill, caution, and responsibility. After all, medical mistakes may make for gripping TV, but in real life, the stakes are much higher.

Methodology

To learn more about the popularity of fictional doctors around the U.S., we compiled a list of 50 recognizable fictional characters from television shows, movies, books, and other popular media with “doctor” in their title. We didn’t limit our analysis to medical doctors alone, but included fictional characters more broadly as long as they were known as “Doctor” or “Dr.” 

We then analyzed regional interest in each character using Google Trends search interest from April 2025 to April 2026. To determine the most popular fictional doctor in every state, we compared search interest for each character head-to-head across all 50 states. We used search interest as an indicator of popularity for this study.
